My mates trying to play online on his PS3, it keeps asking him to input a PSN card number or promotion code. 
 
What's the crack? I've never had to do this before?

Sounds like he's doing something wrong. Maybe he's trying to sign up for a PS+ subscripition, which is a paid service!? 
 
If he has a PSN account, all he has to do is log in. 
 
If he doesn't, creating a PSN account doesn't require a credit card.

Ok, so it turns out that for this game (MLB: The Show 13) you needed to input a code that's inside the box on a loose bit of paper. My mate put that code in, it was a brand new sealed game, and the code had apparently been used already. Where does he stand on this?

No, it's a Sony title. 
 
No offence to your mate, but sounds like he has no clue what he's doing. Probably he already used the code himself? Possibly on a different account? 
 
If he's certain he didn't use the code, contact Sony and they should be able to see when the code was claimed. If it's prior to the purchase date on your mates receipt, they'll give him a new one.

He bought the game, but didnt open it as he bought his son a PS3 for xmas and didnt have a PSN account before this time, he opened the game, loaded it, put the code supplied in but it was already redeemed.

Did he get the game from Amazon? I know more than one person who has worked for them and wrote the numbers down before boxing it up.

He lives in Australia, but he did buy it from Amazon UK though and got it shipped international. It was sealed when he got it though.
