Starting could of been flooding, my GSi has done it before, keeps turning over and over, eventually starts, runs rough, clears (with a puff of smoke) and runs ok after that.
As with most things if it keeps doing it look into it further, if it stops then i shouldn't worry (could honestly be a million and one things which if you were to start looking for could get very expensive and time consuming!)
Gear selection, well it's all mechanical, so first thing I'd look at would be clutch wear as generally the gearboxes are pretty stong and linkages don't tend to be a problem with wear - only thing that does wear is the clutch so maybe investigate that further?
