Yeah, I killed him last night! When can I try the Insane mode? Do I have to complete it in Hardcore first?

Insane mode gets unlocked when you complete the game on any difficulty.

also, if you now play through on Insane (which I suggest you do) you'll unlock the achievements for Hardcore as well
