This is what I put and it did the job

Dear *****,

Please accept this as formal notice of my resignation from *****, with effect from *****.
I am happy to continue to work until *****.

While I believe that I am moving for good reasons, I am sorry to leave, and I thank you for your support during my time with the company.

write on a sheet of plan A4 paper "my resignation" and then walk to you boss and say "can i hand this in to you please"
