Do highway insurance require an engineers report for a 1.0 12v to 1.4 16v conversion?

not an engineers report just a form that you fill in with all relevant details...

flux and brentacre use highway as underwriter, and as mav says you just fill a form in. I had to proove cat 1/2 to them also
