well this morning for some reason i decided it would be a good idea to plug 220v ac into the 5v dc input on a motor controller. it went boom! costs about 3k for a new card! woops

broke someone's finger at work,worked for a removal company and we were lifting a dresser over stairs and a banister and i moved forward a lil bit and trapped his finger between a massive dresser and the wall....
in my defense he was holding it wrong so technically it was his fault  lol

We deliver sport/news data to companies worldwide in XML format, one of our clients is based in Vietnam and each year they requested that we sent the yearly EPL fixture file to them on a CD rather than FTP'ing it (some stupid security protocol of theirs), so every year when the fixtures are released, we generate them and burn them to CD, stick it in the mail and thats that for another year, except I put a blank CD in the envelope by mistake.
A few weeks later we find that the courier who delivered it had been arrested and interrogated for 5 days as the vietnamese company thought we were taking the piss out of them by sending them a blank CD and charging them like £20,000 for it 
It didn't help when someone at work got a picture of some poor bloke with no legs and one arm dragging himself around the streets of ho chi minh on a skateboard and started rumours that it was the courier after his interrogation

i drilled into an electrical trunking without placing a wooden block inside to protect the cables. shorted out the main supply into one of our buildings and the entire dept had to be isolated for a day whilst we rewired it. estimated loss of $94,000 in production. i was not in the good books but they got over it.

Deliverered two boats to Ireland but because I fucked up the maths and put the Single Point Lifting frame in the wrong place [100mm Out] meaning that when you lifted the boat it hung with the nose 12.5 degrees down.
We had to bring them back at our own cost and time and put them right. Each one cost about £5000 to put right

Nearly burnt down a whole cruise ship with 2500 snotty americans on it. I run the gyms on 5 star cruise ships. Anyhoo i woke up and opened up as usual, i had to plug in water heaters that warm volcanic stones for massage therapists. Anyway stupid bitch took the water out of her heating tank (she shouldnt have, and they never do this) so as normal i come in switch it on and go. Went for breakfast and came back to my manager saying the fire crews have just been and put a fire out in the massage room, after the bridge was alerted thanks to the smoke alarm in the room.

dropped 4 flywheels  cost about 100 quid each and they can't be re-used
one dropped on my oil canister for my air-gun and broke it, oil pissed out everywhere and the line had to stop for 30 minutes (it's £1,000 a minute) so ford lost out on £30,000 because of me
well, in my defence, the button i have to press to send the engine on broke and it sent itself automatically, so i was sabotaged
