Ideally yes. K&N use pre-filters on some of their products to stop dirt getting onto the filter... makes you wonder what good their filters are like without the pre-filter 
Because (Pipercross) foam has a number of layers, it's a pre-filter and filter all in one.
More layers = greater capacity for dirt retention = longer service intervals = a filter that will supply the engine with air for longer.
All that's left to do is get the fine balance between the number of layers of foam, the ppi or density of each layer and the thickness of each layer.

Not really, cotton-gauze (oiled) is a very good filter as it stops even tiny micron particles getting through and still flows air.
The problem is that it's just a single layer (hence the pleats - these help to increase the surface area) and it will clog faster than other filters, typically three times faster than a foam air filter.
This is why you get a lot of people saying that replacing a paper filter more often is more economical than fitting a cotton-gauze filter (panel, cone, whatever). In fact, paper will offer better performance at a given level of dirt load than a cotton-gauze filter.
All filters either need to be cleaned or replaced. If they're cheap like paper, just replace them. If they're an investment like a performance filter should be, then clean it.
