If you go to your nearest DVLA office, they will take the private reg off the car there and then (new tax disc etc) for £100 ish (depending if you are putting reg on retention or putting straight on to another car). New V5C might take 2wks to arrive, no more I'd say (from previous experience).

Best to take it off BEFORE you sell though, otherwise technically you've sold the plate with the car and aren't entitled to it back.

Could I swap Car A's plate to car B while also swapping a new reg to car A??

Bloody hassle. I bought an S Works, the bloke wanted to keep the V5 and sort it out then send it to me (with me buying the car with no V5, yeah right) so I signed an agreement saying I'll buy car as is and transfer plate back to him once I have the V5 back. All I got was the badger moaning and pestering me all the time about some shitty plate. I'd only buy a car with a private plate on if it was INCLUDED from now on!
